目的 探讨高度近视眼白内障人工晶体植入术后眼轴变化的相关因素及其影响。方法 统计分析超声乳化人工晶状体植入手术后,64眼(52例)高度近视眼的眼轴变化及其它随访结果。平均年龄62.3岁,屈光度均≥-6.0D,术前平均眼轴长度28.24±2.233mm,平均应植入人工晶状体屈光度数7.5±4.7D,术前矫正视力;光感至0.8。随访时间最短2月20天,最长18月26天,平均7月09天。结果 术后被复测52只眼(占64眼的81.3%),其术前平均眼轴长度为28.39±2.16mm,术后平均眼轴长度27.52±2.230 mm,平均短于术前眼轴0.77±0.472mm,术后眼轴显著短于手术前(P﹤0.001),眼轴长度与视力无显著关系(P﹥0.05),术后所有患者的矫正视力较术前提高,其中24只眼≥1.0(37.5%), 52只眼≥0.5(81.2%)有非常显著提高(P﹤0.01),无并发症,无视网膜脱离,术后眼底病变经治疗后有好转并视力提高。结论 高度近视眼白内障术后,眼轴有自行缩短的倾向,眼轴的变短有利于眼底视网膜组织的稳定。

To analysis the axial length changed by postoperative phacoemulsification and intraocular lens implantation for high myopia with cataract

Abstract Objective To explore the effect and the related factors of the axial length changed by postoperative phacoemulsification and intraocular lens implantation for cataract with high myopia.
Methods Statistically analyse the changes of axial length and other results of 64 eyes (52 cases) were performed by phacoemulsification and intraocular lens implantation for cataract with high myopia. Mean age 62.3 years. All cases of the dioptre ≥6.0D , follow-up from 2 months and 20days to 18 months and 26 days , mean 7 months and 9 days . Mean axial length were 28.24±2.233 mm in Preoperative, their intraocular lens of dioptre mean +7.5D±4.7D by detected. The acuity vision was in light sense to 0.8.
Results Catched the 52 eyes (81.3%) were measured the axial length is 27.52±2.230,that they were shorter than Preoperative axial length of 28.39±2.16mm in their average shorten 0.77±0.472mm, (P﹤0.001). It is significantly to see that all the cases acuity vision has been improved after operation, 24eyes ≥1.0 (37.5%),52 eyes ≥0.5(81.2%), (P﹤0.01) , no complication , no retina detachment also, The further more, the vision is improved by therapy because there are some lesion on fundus of eye to show with ophthalmoscopic in postoperative.
Conclusion It is helpful and resistant for retina tissue due to the axial length shorter spontaneously after phacoemulsification and intraocular lens implantation of cataract with high myopia .
