Purpose: To describe the disease presentation and visual outcome of Behcet’s disease (BD) with uveitis during a twenty-year period in a defined area of Taiwan. Methods: 227 patients with uveitis caused by BD who visited the Uveitis Service, Departement of Ophthalmology, Taipei Veterans General Hospital between 1984 and 2003 were enrolled. The demographic data, extraocular manifestations, HLA-B51 association, and visual outcome were analyzed. Comparisons between patients who presented in the first decade and the second decade were also conducted. Results: 140 patients (61.7%) were male and 87 (38.3%) were female. The average age of onset was 30.810.9 years. A remarkable difference was found in age of onset by decade, with peak incidence in the 3rd and 4th decades for male patients and an even distribution for females. HLA-B51 antigen was found in 35.7% of 143 detected patients. Sex ratio, age of onset, and association of HLA-B51 did not differ between groups. The most common extraocular manifestations were oral ulcer (97.4%). Extraocular symptoms did not differ between groups and sexes except more skin lesions in group A and male patients. 84.2% had bilateral ocular involvement, which was decreased in group B (P=0.04). Kaplan-Meier survival analysis estimated the risk of loss of useful vision to be 43.0% at 10 years. The mean time from presentation to loss of useful vision was 51.040.3 months. The visual prognosis was better in females. Although Kaplan-Meier survival analysis demonstrated no significant difference between groups, the visual prognosis was better in group B with fewer patients losing their useful vision, more patients having final visual acuities better than 0.5, and the slower rate of visual deterioration. Conclusion: Male patients were associated with poor visual prognosis. And the visual prognosis was better in the last 10 years. The number of patients was not decreased in the recent ten years.
