Prevalence of age-related cataract and blindness in different altitude regions of West China: “Lifeline Express” Cataract Study

Purpose. To evaluate the prevalence of age-related cataract and cataract blindness among adults ages 50 or older in three different altitude regions of West China to delineate the overall prevalence of age-related cataracts in rural areas of West China. Methods. This study uses the method of cluster sampling and recruitment of participants through household census. All three established test sites are located in rural areas. The sample size is 4324 people ages 50 years and above. The altitudes of the three areas are 2300m (Qinghai), 1857m (Yunnan) and 106m (Guangxi). Standardized ocular examinations were performed to assess age-related cataracts. Cataract blindness is defined as that which presents with a bilateral visual acuity of less than 6/60. Results. The overall response rate was 89.3%, and 86.2%, 91.4%, 90.4% for Qinghai, Yunnan and Guangxi respectively. The prevalence of age-related cataract was 41.84% in Qinghai, 37.7% in Yunnan and 39.42% in Guangxi. The prevalence of age-related cataract increased with age. The age-adjusted prevalence of age-related cataract correspondingly changed to 44.07%,43.68% and 32.78% under the data of China census 2000. The prevalence of blindness due to age-related cataract was 4.13%, 7.39% and 4.36%. The highest Cataract Surgery Rate (CSR) value was 1551 and the lowest was 143 among these three rural regions during the period 2000~2002. Conclusions. The prevalence of age-related cataract and blindness aged 50 and above was different in different altitude regions of China. The high prevalence of age-related cataract of Chinese rural population must be considered. Age-related cataracts remain a major reversible public healthcare problem in western China. Reducing the amount of cataract blindness in China through surgery is imperative.
