Aims: To compare the in vitro killing effect of different agents on demodex and to report the in vivo killing effect of Tea Tree Oil (TTO) on ocular demodex. Methods: Survival time of demodex was measured under the microscope. Sampling and counting of demodex was performed by a modified method. Results: : Demodex survived in 10% povidone iodine, 75% alcohol, 50% baby shampoo, pre-treated 50% baby shampoo or 10% SDS for 30 min before 10% povidine iodine, or pre-incubated 10% povidone iodine for 30 min before 75% alcohol for more than 150 min. However, demodex survival time in 100% alcohol was 3.9 ± 1.2 min, in 100% TTO was 3.7 ± 0.8 min, 100% caraway oil was 4.4 ± 2.3 min, in 100% dill weed oil was 14.0 ± 8.3 min. The in vivo killing effect of demodex in TTO is dose-dependant and the survival time in 50%, 25% and 10% TTO lengthened to 14.8 ± 9.5, 34.7 ± 4.3, and 150 min, respectively (p<0.01). None of the 7 patients received baby shampoo daily lid scrub drop to zero at the duration of 40 to 350 days. Among the 9 TTO treated patients, demodex count drop to 0 within 4 weeks in 7 of them, 2 didn’t finally reach 0 at 35 and 63 days after treatment relatively. Conclusions: Demodex is resistant to a wide range of solutions but susceptible to some including tea tree oil, caraway oil and 100% alcohol. Weekly lid scrub with 50% TTO and daily lid scrub with tea tree shampoo is effective in eradicating ocular demodex.
