Effect of Astigmatism on Accuracy of Accommodation in Children

PURPOSE: Accommodation reflects both distance and ametropia. Children partially accommodate, exhibiting accommodative lag (AL). Astigmats perceive additional blur at the same AL. We studied this interaction.
METHODS: Subjects (Native American (Tohono O’odham)) with prevalent uncorrected astigmatism (cyl) viewed a cartoon 50 cm away. Cyclopegic (Nikon (now Right) Mfg Retinomax K-Plus) and non-cycloplegic right eye refractions (Shack-Hartmann Sensor (SHS)) were made. Multiple SHS images were recorded, reviewed and analyzed to determine AL. Blur, including both AL and cyl power vectors, was calculated.  Correlations were performed.
RESULTS: 57 children (3 to 7 years) had average cyl of 1.06D (range 0 to 4D). Cyl was WTR with vertical Jackson Cross Cylinder (JCC) J0 mean of 0.481 D, and oblique JCC J45 of -0.004D. Average AL was 1.14 D (range -0.22 to 3.12D).  Correlations of lag, blur, age, SEQ and cyl were performed. Age was not correlated with lag (p=0.182), while cyl did (p=0.028), with marginal significance (p=0.059) after adding SEQ to the model. Blur (power vector of AL, J0 and J45) remained correlated with SEQ (0.007).
CONCLUSION: Children experience both astigmatic and AL blur.  Models should incorporate both.
